Product Description:
ZEMICOFUNG® – V is an injectable formulation of Voriconazole, a broad-spectrum second-generation triazole antifungal used for the treatment of serious invasive fungal infections, including those caused by Aspergillus, Candida species, and Scedosporium.
It is considered the first-line treatment for invasive aspergillosis and is used in neutropenic patients, transplant recipients, and ICU patients at high risk for systemic fungal infections.
Key Benefits:
-
Rapid and reliable IV delivery of broad-spectrum antifungal action
-
Highly effective against azole-resistant Candida and non-albicans species
-
Superior tissue penetration, including CNS and pulmonary tissues
-
Proven efficacy in immunocompromised and critically ill patients
How It Works:
Voriconazole works by inhibiting fungal cytochrome P450-dependent 14α-sterol demethylase, a key enzyme involved in ergosterol synthesis, which is essential for maintaining fungal cell membrane integrity. This inhibition leads to fungal cell death.
How to Use:
-
Reconstitute the lyophilized powder with 19 ml of sterile water for injection to yield a clear solution
-
Further dilute in 100 ml of 0.9% NaCl or 5% dextrose for IV infusion
-
Recommended adult dosing:
-
Loading dose: 6 mg/kg every 12 hours for the first 24 hours
-
Maintenance dose: 4 mg/kg every 12 hours
-
-
Infuse over 1–2 hours, under strict medical supervision
-
Adjust dose in patients with hepatic impairment
Safety Advice:
-
Monitor liver function tests, electrolytes, and serum drug levels during therapy
-
Avoid concurrent use with certain CYP3A4 substrates, like sirolimus or rifampin
-
Use caution in patients with pre-existing arrhythmias or QT prolongation
-
Temporary visual disturbances (blurred vision, photophobia) are common and reversible
Precautions:
-
Not for intramuscular or bolus IV injection
-
Contraindicated in:
-
Known hypersensitivity to voriconazole or triazoles
-
Co-administration with certain drugs (e.g., ergot alkaloids, carbamazepine)
-
-
Use with caution in:
-
Pregnant or breastfeeding women (only if benefit outweighs risk)
-
Hepatic and renal impairment (monitor closely)
-
-
Avoid sunlight exposure; photosensitivity reactions have been reported
-
Reconstituted solution should be used immediately or stored in the refrigerator (2–8°C) and used within 24 hours
